Patient Access to Online Records

The NHS wants to give people better ways to see their personal health information online. We know that people want to be able to access their health records. It can help you see test results faster. It also lets you read and review notes from your appointments in your own time.

We're now letting you see all the information within your health record. If you are over 16 and have an online account, such as through the NHS AppNHS website, or other GP online services and apps, you will now automatically be able to see all future information entered on your medical record. Some people can already access this feature, this won't  change for you.

This means that you will be able to see records from your appointments, as well as test results and any letters that are saved on your records. This only applies to records from your doctor (GP), not from hospitals or other specialists, although you will be able to see any letters that hospitals or specialists send to your doctor (GP). You will only be able to see information from 31 October 2023. For most people, access will be automatic, and you won't need to do anything.

Your doctor (GP) may talk to you to discuss test results before you are able to see some of your information on the app. Your doctor (GP) may also talk to you before you are given full record access to make sure that having access is of benefit to you. If you are concerned that seeing your records is not right for you, you should speak to your practice.

These changes only apply to people with online accounts. If you do not want an online account, you can still access your GP health record by requesting this information via reception . The changes also only apply to personal information about you. If you are a carer and would like to see information about someone you care for, speak to reception staff.

The NHS App, website and other online services are all very secure.  You'll  need to make sure you protect your login details. Do not  share your password with anyone as they will then have access to your personal information.

We would encourage you to use online facilities to order repeat prescriptions and manage appointments where you can. 

We have now moved to a new clinical system called Systmone. This means that the old online service is no longer in service. To regain online access you will need to either download the NHS app or reapply for online access using the forms below.

We have various options for patients to access online information:

The NHS APP – Owned and run by the NHS, the NHS App is the simple and secure way to access a range of NHS services on your smartphone or tablet.

Use the NHS App to:

  • book appointments – search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments
  • order repeat prescriptions – see your available medicines and request a new repeat prescription
  • check your symptoms – search trusted NHS information and advice on hundreds of conditions and treatments, and get instant advice or medical help near you
  • view your medical record – securely access your GP medical record
  • register your organ donation decision – choose to donate some or all of your organs and check your registered decision
  • find out how the NHS uses your data – choose if data from your health records is shared for research and planning

Online Patient Access –  This is set up using your email address and a password generated by the surgery.
You will need to come into the surgery to complete a registration form or you can print a copy of the form below then bring it to the surgery; you will be asked to provide two forms of identification to the member of staff,  one form on photo identification and one form of identification that confirms your address. Please note that you do need to come in person with your photo id, we cannot accept someone bringing this in on your behalf. Where you are applying for proxy access for a patient aged 11-16 the child must attend the surgery with the person requesting the access to give verbal consent for their record to be accessed.
