Online 'Pre-Registration' With The Practice

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. You can also scan the completed form and email it to us at admin.miltonsurgery@nhs.net

You will need to complete the Registration form (GMS1 form) and the appropriate health questionnaire (there is a separate one for new born babies) from the forms below. These can be printed off and completed before you come to the surgery. The forms can also be emailed to the above email address along with your registration form.
