New Patient Registration

We are happy to take on new patients in the Milton and Landbeach area.  The practice does not have capacity at this time to register new patients who live outside the practice boundary nor does the practice have capacity to register existing patients to any area not listed in the practice leaflet.

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are required to have health check with a member of the healthcare team to ensure that any required tests are up to date.

If you are taking regular medication, new patients are required to see a doctor before any repeat medication can be prescribed.

Medical treatment is available from the date of registration. Please contact reception for further information.

Allocated GP:  All existing patients not already allocated a named GP will have a doctor allocated to co-ordinate their care by 30 June 2015.  To find out who your named doctor is please ask a receptionist.  You remain free to see any doctor at the practice.
